Job summary

BrightSpring Health Services, in Elizabethtown, KY, is seeking a dedicated Home Health RN to join our team. This role offers 1:1 patient care, autonomy with team support, and opportunities for growth within a growing company.

The Home Health RN will assess patients, create care plans, administer medications, and educate families while collaborating with an interdisciplinary team to ensure high-quality, compassionate care in the patients’ homes.

Qualifications

• Must hold a registered nursing license in the state of practice and current in Good Standing.
• Minimum of 1 year nursing experience in patient care; home health or geriatrics preferred.
• Valid driver’s license with acceptable driving record; proof of auto insurance as per policy.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Develop and implement home health care plans aligned with MD orders and patient goals.
• Provide direct nursing care including medications, treatments, and interventions.
• Educate and support patients’ families and caregivers.
• Collaborate with an interdisciplinary team and document accurately and timely.
• Participate in on-call rotation as required by the branch.

Responsibilities

As a Home Health Registered Nurse, You will:

• Assess/monitor physical, emotional, and psychological needs of patients

• Educate and support the patient's family and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch

Qualifications

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required
• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred
• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• New nursing graduates may be considered in select markets based on program availability
• Current CPR certification
