Job summary

Personal Touch Home Care is seeking a Registered Nurse for home health care in Jefferson County, IN, covering Madison and Hanover. Part-time or full-time positions are available with cases based in Jefferson County.

Qualifications

• Graduate of an approved nursing program or Bachelor’s degree in Nursing.
• Currently licensed to practice as a Registered Nurse.
• Minimum 1 year of community health nursing or equivalent experience.
• Knowledge of home care nursing practices and OASIS.
• Complies with professional standards and practice.

Responsibilities

• Assessment and Care Planning: conduct patient assessments and develop care plans.
• Medication Administration: administer prescribed medications and monitor responses.
• Wound Care: manage wound care including dressing changes and infection control.
• Patient Education: educate patients and families on conditions, medications and self-care.
• Monitoring and Evaluation: track progress and adjust care plans as needed.
• Collaboration: coordinate with physicians, therapists, social workers, and aides.
• Documentation: maintain medical records and nursing assessments.
• Support and Advocacy: provide emotional support and uphold patient rights.

JOB SUMMARY

• Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs.

• Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects.

• Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress.

• Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home.

• Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes.

• Collaboration: Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient.

• Documentation: Maintain accurate and up-to-date medical records and documentation of nursing assessments, interventions, and patient responses.

• Support and Advocacy: Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.

QUALIFICATIONS

• Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university

• Currently licensed, in good standing, to practice in Kentucky as a Registered Nurse.

• Minimum 1 year community health nursing experience or equivalent professional experience.

• Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS.

• Complies with accepted professional standards and practice.

Job Types: Part-time or Full-time

Salary: Up to $80,000.00/annually
