Qualifications

• Licensed RN required with strong clinical, assessment and organizational skills.
• Home health experience preferred but training available.
• Background checks and references required; CPR certified.

Responsibilities

• Provide home health nursing care to patients in their homes.
• Perform patient assessments and develop care plans in collaboration with the team.
• Maintain a set daytime schedule with weekends and holidays off with pay.
• Coordinate with interdisciplinary team to ensure quality patient care.
