Live Well Homecare is seeking an experienced registered nurse to join an interdisciplinary team, promoting preventative care, conducting screenings, coordinating disease management, and supporting clinical decisions.

You will assess patient needs, develop individualized care plans, educate patients and families, and collaborate with providers to adjust therapies. The role emphasizes patient-centered care and quality improvement.

Qualifications

• RN with a minimum of two years' Med/Surg experience.
• Graduate from Specialty Nursing Training Program (required).
• Bachelor's Degree in Nursing preferred.
• Strong clinical, communication, and teamwork skills.

Responsibilities

• Assess healthcare, educational, and psychosocial needs with patient and family involvement.
• Coordinate diagnostic tests and maintain documentation in electronic systems.

Role Overview:

This position involves serving as an expanded nursing professional within an interdisciplinary team to promote preventative care, conduct screenings, coordinate disease management, and support clinical decision-making. The nurse will assist with triage, assess patient needs, develop individualized care plans, and facilitate communication among patients, families, and healthcare providers.

Key Responsibilities:

• Assess healthcare, educational, and psychosocial needs, involving patients and families in treatment planning.
• Investigate healthcare options, facilitate communication, and eliminate barriers to treatment.
• Educate patients and families on disease management, care processes, and case management outcomes.
• Implement clinical interventions based on risk and evidence-based guidelines, collaborating with providers to adjust therapies and monitor progress.
• Coordinate diagnostic tests and maintain documentation in electronic systems.
• Participate in professional development and incorporate current literature into practice.
• Support care transition processes and assist in designing, implementing, and evaluating care programs.
• Perform clinical procedures such as specimen collection, IV management, and medication administration under provider supervision.

Qualifications & Skills:

• Minimum of two years RN experience, preferably in Med/Surg settings.
• Graduate from a Specialty Nursing Training Program (required); Bachelor's Degree in Nursing (preferred).
• Strong clinical, communication, and teamwork skills, with a focus on patient-centered care and evidence-based practice.
