Job Description:

• Complete assessments as per scope of practice and standards.
• Address physical, psychological, social, cultural, and spiritual needs of patients and families throughout care.
• Develop, coordinate, and implement care plans including education and discharge planning.
• Provide direct patient care including treatments, medications, and diagnostics.
• Evaluate and revise care plans based on patient responses.
• Collaborate proactively with physicians and team members to meet patient needs.
• Manage equipment and troubleshoot as needed.
• Participate in evidence-based practice decisions.
• Contribute to team goals and organizational metrics.
• Engage in quality and safety education and initiatives.
