Job description

Job Title: Registered Nurse (RN) - Home Health

As a Phoenix RN, you will be the first point of contact between patients and the hospital. Our Home Health RNs should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized care plan based on physician orders and initial assessments. You will work alongside other nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.

Responsibilities

• Develop, review, and revise the care plan
• Provide nursing care according to physician’s treatment plan and individual care plan
• Observe, record, and report reactions to treatment and any changes in patient's condition
• Supervise and instruct clients, families, and health team members in treatment plans
• Participate in education programs, quality assessments, and staff meetings

Requirements

• Active RN license in Missouri
• Home Health experience required
• Valid driver’s license, vehicle, and current auto insurance
• Strong organizational and self-management skills
• Excellent communication skills
