About the Role: Licensed Practical Nurse

As a Registered Nurse with Complete Care at Barn Hill, you will be responsible for:

Responsibilities as an RN: Licensed Practical Nurse

Conduct initial patient assessments, identifying medical, emotional, and social needs
Develop and implement individualized care plans, prioritizing patient goals and outcomes
Implement and evaluate the effectiveness of care plans, making adjustments as necessary
Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
Participate in quality improvement initiatives, contributing to best practice development and program evaluation
