Job summary

Hope Creek Nursing and Rehabilitation Center in East Moline, IL is seeking a full-time Registered Nurse (RN) to provide direct patient care and develop individualized care plans. You will collaborate with interdisciplinary teams and monitor patient progress to ensure high-quality outcomes.

Qualifications

• Bachelor's degree in Nursing (BSN) or ADN from an accredited program.
• Current RN license in the State of Illinois.

Responsibilities

• Provide direct patient care, including assessment, planning, implementation, and evaluation of individualized care plans.
• Administer medications, treatments, and therapies as prescribed by physicians.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor and report patient progress, identifying areas for improvement and implementing changes as necessary.
• Participate in quality improvement initiatives and contribute to a positive and supportive work environment.
