Job summary

BAYADA Home Health Care is seeking a Registered Nurse to provide home health visits in Charlottesville, VA. The role requires a VA RN license and at least one year of clinical experience, with preference for prior home health exposure.

You will follow care plans, perform medications and wound care, monitor patients, and document care using an EMR system. Strong independence, time management, and travel ability are essential.

Qualifications

• Current RN license in Virginia (VA).
• At least 1 year of recent clinical (medical/surgical) experience.
• Prior home care experience preferred but not required.
• Graduate from an accredited nursing program (school transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and good written/verbal communication.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Follow a designated care plan aligned with patient needs.
• Make home visits to clients in assigned geographic territories.
• Administer medications, perform wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical or Client Services Manager.
• Follow up with and document doctors' orders accurately.
• Perform client assessments as necessary.
• Coordinate case management and care
• Document observations and interventions using a touch pad tablet.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in VA.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:
Base Pay, depending upon qualifications:
$55-$60 per point
