Job summary

Healthmap Solutions is seeking a Registered Nurse Quality Practice Advisor to coordinate performance improvement activities that educate and engage providers, aiming to improve health outcomes for members. You will partner with Care Navigation to enhance the Kidney Health Management program and ensure NCQA-compliant documentation.

The role requires RN licensure, BLS, and mandatory travel within the NYC region.

Qualifications

Bachelor’s degree required.
Active, unrestricted RN license required.
BLS certification required within 30 days of hire.
3 years of experience in a health care or managed care setting.
3 years of experience in claims or gap closure campaigns preferred.
3+ years of progressive experience in healthcare services, clinical operations, quality, or care management.
Proof of valid driver’s license and automobile insurance; frequent travel within assigned region.

Responsibilities

Coordinate performance improvement activities to engage, educate, and manage improvements for Healthmap Solutions members.
Partner with Care Navigation and providers to identify and close care gaps.
Educate providers on HEDIS measures and best practices.
Develop and maintain documentation compliant with NCQA standards.
Deliver timely reports to internal and external stakeholders.
