Job summary

Nascentia Health is seeking a Registered Nurse Quality Assurance Coordinator to support quality improvement for the CHHA and LHCSA divisions. You will review records, assist with OASIS-related education, and monitor documentation accuracy across home care services.

The role emphasizes adherence to agency policies, patient confidentiality, and collaborative work with clinical staff and management to drive quality outcomes.

Qualifications

• Current New York State licensed Registered Nurse.
• Minimum 2 years working in home care.

Responsibilities

• Reviews clinical records and participates in chart audits for LHCSA and quality improvement.
• Follows up with staff and management on issues identified in the review process; supports education and documentation issues.
• Performs quality improvement activities assigned by the Manager of Quality Improvement and communicates policies to staff.
• Maintains knowledge of agency policies, quality standards, OASIS assessments, and payer requirements (Medicare/Medicaid).
• Keeps Manager of Quality Improvement informed of problems; participates in meetings and training; performs other duties as assigned.

The RN (Registered Nurse) Quality Assurance Coordinator is responsible for assisting in overall quality improvement activities for Nascentia Health CHHA and LHCSA divisions.

Job Requirements:

• Reviews clinical records.
  • Participates in review of chart audits utilizing SHP reports (if applicable) and best practice following policies and procedures.
  • Responsible for completing all LHCSA audits
  • Reviews client records, orders and other communication for quality and safety of care.
  • Analyzes documentation and reports trends to Manager
• Follows up with staff/management regarding issues identified in review process.
  • Assist clinical leadership in working with clinical staff on questions, OASIS education/clarification, documentation issues, and clinical care interventions.
  • Communicates with Manager regarding issues or concerns with clients and/or clinical staff.
  • Follows up with staff regarding documentation deficiencies and/or discrepancies with OASIS, coding and assessment.
  • Participates in formal and informal training opportunities for clinical staff.
• Performs quality improvement activities as assigned by Manager of Quality Improvement.
  • Communicates agency policies to staff.
  • Promotes quality improvement as it relates to the activities of clinical staff in accordance with organizational policies and procedures.
• Maintains knowledge of and adherence to agency policies, procedures, along with quality improvement standards.
  • Demonstrates knowledge and use of agency policies and procedures.
  • Demonstrates knowledge of quality improvement standards.
  • Demonstrates knowledge of OASIS assessments.
  • Has a working knowledge of Medicare/Medicaid and private insurance
  • Ability to learn and apply coding principles to home care.
• Other expectations

Requirements:

• Current New York State licensed Registered Nurse
• Minimum 2 years working in home care.
• Excellent organizational skills.
• Good attention to detail and accuracy.

• Ability to work independently and be self-directed.

Physical Requirements:

1. Speech/visual/hearing ability sufficient to comprehend and express written and verbal communications.

2. Ability to sit/stand at desk for 90% of the day.

3. Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids.
