RN Psychiatric Case Manager - Dublin, OH

We are looking for qualified RN Psychiatric Case Managers to provide one‑on‑one psychiatric and skilled nursing care to patients in their homes. The role involves evaluating patients’ medical, cognitive, emotional, psychosocial and psychiatric needs and coordinating care under the direction of a physician or psychiatrist.

Responsibilities

• Assess and document psychiatric nursing needs of patients.
• Develop and revise treatment plans and interventions.
• Conduct regular reviews of treatment plans, goals and discharge summaries.
• Coordinate care with multidisciplinary teams, ensuring appropriate services are provided.

Qualifications

• Current, unencumbered license to practice as a Registered Nurse in Ohio.
• Minimum of one (1) year of clinical RN experience.
• CMS requirement for psychiatric nursing in home care:
  • Master’s degree in psychiatric or mental health nursing, OR
  • Bachelor’s degree in nursing with at least one year of recent experience in a psychiatric/mental health hospital or outpatient clinic.
• Current CPR certification.
• Current driver’s license.
• Preference for candidates with previous home health or hospice experience.
