Job summary

A veteran-led healthcare provider in Espanola, New Mexico, is seeking a skilled Registered Nurse to join their team. You will provide high-quality healthcare to Veterans as part of a collaborative Patient Aligned Care Team. The ideal candidate will have at least 1 year of Primary Care or Chronic Disease Management experience and a valid Nursing License. This full-time, onsite role offers an excellent benefits package including medical plans, 401(k), and paid time off.

Qualifications

• Minimum of 1 year of experience in Primary Care or Chronic Disease Management.
• Current, active, and unrestricted state Nursing License is required.
• BLS certification from the American Heart Association, renewed annually.
• Strong computer skills including EMR experience.

Responsibilities

• Provide high-quality healthcare to Veterans as part of a Patient Aligned Care Team.
• Assist practitioners with patient care and triage urgent patients.
• Educate patients and families on self-management and wellness.
• Maintain a welcoming atmosphere for colleagues and Veterans.
