Job summary

VNS Health is seeking a Registered Nurse to practice in the community, delivering personalized nursing and care management to patients in their homes or in care facilities. You will work with an interdisciplinary team and continually evaluate patient needs to adjust plans of care.

Four years of clinical home care experience is required, including two years full-time and at least one year as a field staff nurse. A NY license and degree in nursing are essential.

What You Will Do

• Practice independently in the community as part of an interdisciplinary care team.
• Deliver personalized nursing and care management to patients in their home or care facilities.
• Constantly evaluate evolving patient needs and respond with plan of care adjustments.

Qualifications

Licenses and Certifications: A license and current registration to practice as a Registered Professional Nurse in New York State required.

Education: Bachelor's Degree in Nursing OR Associate’s degree in Nursing required.

Work Experience: Four years’ clinical home care experience required; Minimum of two years full-time clinical home care experience required; Minimum of one year of experience as a field staff nurse in VNS Health Home Care, which includes ability to manage/document cases with a minimum of supervision, identify/assess/formulate/implement action plan to address problem and evaluate outcome preferred.

Pay Range

USD $126,413.00 - USD $160,188.00 /Yr.
