Qualifications

• Active and unencumbered WI RN License or Compact RN License.
• Minimum 1 year of experience in skilled nursing.
• Must have reliable transportation to assigned facilities.
• Must maintain consistent attendance and punctuality.
• Able to push, pull, and lift at least 50 pounds.
• Must be legally authorized to work in the United States.

Responsibilities

• Utilize EMR Matrix to record patients’ medical information and vital signs.
• Assess resident health status and identify changes requiring clinical intervention.
• Administer medications, treatments, and other nursing interventions per physician orders.
• Develop, implement, and evaluate individualized resident care plans.
• Monitor resident outcomes and communicate changes to physicians and team.
• Perform skilled nursing procedures within scope of practice and facility guidelines.
• Document assessments, interventions, and resident responses in the medical record.
• Supervise and support LPNs, CNAs, and other care team members as appropriate.
• Maintain compliance with facility policies, infection control standards, and regulatory requirements.
• Communicate concerns to onsite supervisor and agency.
• Other duties as assigned
