Job summary

AdventHealth in Colorado seeks a Registered Nurse to compile data, track outcomes, and coordinate post-acute care transitions. You will develop discharge plans with physicians, care managers, and families, guiding patients to home health or hospice as appropriate.

You will educate patients and families about post-acute services, maintain documentation, and participate in multidisciplinary rounds to support quality care while upholding the faith-based mission.

Qualifications

• Associate's Degree in Nursing required.
• Bachelor's Degree in Nursing preferred.
• Registered Nurse (RN) licensure required.
• Basic Life Support (BLS) certification required.
• 1+ year of clinical experience in a relevant area.

Responsibilities

• Compiles data, tracks outcomes, and makes recommendations for process improvement.
• Develops and maintains relationships with physicians, post-acute providers, therapists, patients, and families.
• Collaborates with care management, physicians, and clinicians to develop discharge plans requiring post-acute services.
• Ensures seamless transitions across the continuum of care.
• Educates patients and families about post-acute care services and expectations.
• Assists in assessing patients for post-acute care and coordinates the clinical transition to home health or hospice as appropriate.
• Maintains knowledge of managed care and community resources.
• Completes and submits all documentation in a timely manner.
• Participates in multidisciplinary rounds, care conferences, and coordination with case management.

Maintains comprehensive knowledge of managed care and community resources. Completes and submits all documentation in a timely manner according to department policy. Participates in multidisciplinary rounds, care conferences, and coordination with case management. Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers. Coordinates efforts for treatment and promotes quality improvement and educational efforts. Provides referrals and supports for follow-up care for patients who are uninsured or underinsured. Other duties as assigned.

Knowledge, Skills, and Abilities: Competent with Microsoft Office [Required] Possess strong organizational skills with the ability to prioritize, manage and complete multiple tasks and projects within specified timeframes. Pay Range: $73,611.20 - $136,905.60
