Endeavor Health’s Evanston Hospital is seeking an RN Patient Care Navigator for the Stroke Program. Per diem shifts from 7:00AM to 7:00PM require flexibility. The role involves nursing care for acute stroke patients, coordination with the stroke team, and patient education to support stroke recovery.

The ideal candidate holds a BSN, Illinois RN licensure, and certifications in CPR, ACLS, and NIHSS, with several years of RN acute care experience in stroke settings.

Responsibilities

• Renders professional nursing care for acute stroke patients under physician/APP supervision.
• Assesses conditions, recognizes deviations, and applies emergency protocols as needed.
• Collaborates with the stroke coordinator and neurosciences team to meet center standards.
• Delivers education to patients and care partners about stroke care and recovery.

Skilled in treating deteriorating and emergent patient conditions. Employs critical thinking skills, assesses patient conditions, recognizes deviations, implements appropriate treatment protocols for specific emergencies, evaluates patient response, records observations, and interventions, assists with transport to emergency testing and transfer to higher level of care as indicated. Collaborates with stroke coordinator and neurosciences team to ensure care delivery meets comprehensive stroke center standards.
