Endocrine News is seeking an RN to serve as a Patient Care Navigator for the Stroke Program at Evanston Hospital. This per diem role involves 12‑hour shifts from 7:00 AM to 7:00 PM, with flexibility on days, in a fast‑paced acute stroke care setting.
The RN will coordinate care with neurologists and the multidisciplinary team, provide education to patients and families, and ensure adherence to standards of care and quality initiatives within the stroke program.
Hourly Pay Range: $40.45 - $62.70. The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
The Stroke navigator will render professional nursing care to assist neurologist, physicians, and APPs in coordinating the care of patients throughout the stroke continuum with primary focus on acute stroke alerts, supporting acute interventions, and collaborating with multidisciplinary team to ensure timely evidence‑based care. Ability to effectively function during emergent situations and manage multiple priorities. Employs critical thinking skills, confident neurological assessments, adheres to appropriate treatment protocols for specific emergencies, effectively communicates plan, assists with transport to emergency testing and transfer to higher level of care as indicated. Synergizes with stroke coordinator and neurosciences team to ensure care delivery meets comprehensive stroke center standards including helping maintain stroke program data. Provides stroke education to patients, families, and healthcare staff. Ultimately, this role serves as a clinical resource for stroke protocols, quality improvement initiatives, and regulatory compliance.
