Elite Home Health, part of LHC Group and a member of the Optum family, is hiring a Home Health Patient Care Manager to supervise and coordinate clinical services. You will lead an interdisciplinary team to ensure continuity of high-quality home health care in line with physician plans of care and applicable laws.
You will review assessments and plans of care daily, assign clinicians, and tailor care to each patient’s needs while ensuring compliance and patient safety.

As the Home Health Patient Care Manager, you are responsible for the overall supervision and coordination of clinical services. You coordinate and supervise an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team’s area in accordance with physician prescribed plans of care, and all applicable state and federal laws and regulations.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $85,196 to $127,794 annually based on full-time employment. We comply with all minimum wage laws as applicable.
