  • Monitor and report changes in patient’s condition to a registered nurse or provider.
  • Maintains participants’ medical record and completes Reid Health PACE charting and reporting requirements.
  • Triages participants within scope of practice to determine priority of care.
  • Participates in administering participant vaccinations.
  • Assists other staff in the home setting or clinic during appointments and emergencies.
  • Maintains supply inventory for clinic medication room.

Job description

RN - Part time 0106 PACE Schedule: Day Shift. 24 hours.3 – 8-hour shifts per week. Hours would be 8a-430p.

About the Position Under the direction and supervision of the home care Registered Nurse and Clinical Manager, assists with participant medication management and implementation of nursing services in the home setting as needed for the Reid Health PACE program. This individual works hand in hand with PACE care providers to achieve quality outcomes for the participant. Communicates participant’s condition to primary caregivers, providers, and the interdisciplinary team. Accepts other duties as assigned by the Clinical Manager and within the scope of practice.

Overview of Responsibilities
  • Provide skilled nursing care in the home environment under the direction of a Registered nurse or Provider.
  • Provide basic medical care, including wound care, medication management, and collecting specimens as directed by the Provider.
  • Instructs and educates participants and families on medication and treatment instructions.
  • Monitor and reports changes in patient’s condition to a registered nurse or provider.
  • Ensures chart documentation is completed in a timely manner and in accordance with practice policy and procedure.
  • Performs lab tests in accordance with participant's needs and as directed by the Provider.
  • Prepares specimens for in-house and reference labs as appropriate.
  • Observes, records, and reports participant’s condition and reaction to drugs and treatments to Providers.
  • Must be committed to Participant Satisfaction and Participant Safety.
  • Maintains participants’ medical record and fulfills Reid Health PACE charting and reporting requirements.
  • Triages participants within scope of practice to determine priority of care.
  • Participates in administering participant vaccinations.
  • Documents and records appropriately.
  • Prepares equipment and assists the provider during treatment, examination, and testing of participants.
  • Assists other staff in the home setting or clinic during appointments and emergencies.
  • Maintains supply inventory for clinic medication room.
  • Effectively communicates in clinical meetings or any other meetings as assigned.
  • Support participants across all care settings.
  • Monitors operational compliance with OSHA regulations, Infection Control protocol, and agency policies and procedures.
  • Participates in and supports Quality Improvement initiatives.
  • Participates in continuing education classes, staff meetings and any required training.
  • Maintains professional affiliations and any required certifications.
  • Complies with CMS PACE regulations as they pertain to this role.
  • Performs other duties as required or requested.

Education & Experience
  • Education Required: Licensed Practical Nurse
  • Education Preferred: Licensed Practical Nurse
  • Experience Required: A minimum of one year's experience working with the frail or elderly population.
  • Experience Preferred: 3+ years of experience working in health care facility or practice.
Licensure & Certification
  • Licensure: Current Licensure in the State of Indiana as a Licensed Practical Nurse.
  • Certifications: CPR Certification, IV Certification.
