The Mary Lanking Memorial Hospital Association in Nebraska is seeking an RN-Palliative Care Coordinator responsible for coordinating and facilitating day-to-day operations of the Palliative Care Program to ensure safe, high-quality patient care. This role supports the Palliative Care APRN in care coordination and discharge planning.

Qualifications

• Associate degree in nursing required; bachelor’s degree preferred.
• RN license in NE required; BLS and ACLS certifications required.
• 3 years of medical/surgical or specialized clinical experience; case management preferred.

• Coordinate and facilitate day-to-day operations of the Palliative Care Program to maintain safe, quality patient care.
• Assist the Palliative Care APRN with coordination of care and discharge planning for Palliative Care patients; communicate with hospital staff and families.

Job Description: The RN-Palliative Care Coordinator position Is responsible for coordinating and facilitating day-to-day operations of the Palliative Care Program to maintain safe, quality, efficient patient care. This position will assist the Palliative Care APRN with coordination of care and completion of required admission information, participation in communication with hospital staff and family for discharge planning for Palliative Care patients.

Position Requirements

• EDUCATION: Associate Degree in Nursing required (Bachelor's preferred).
• EXPERIENCE: 3 years experience in medical/surgical or a specialized clinical area required. Case Management experience preferred.
• CERTIFICATE, LICENSES, REGISTRATIONS: Current registered nursing (RN) license from the State of NE required. BLS and ACLS certifications required.
