Job summary

Trinity Health IHA Medical Group seeks a compassionate Registered Nurse (RN) to join our outpatient care team in east Brighton. This part-time role focuses on telephone triage, clinical triage, care coordination, and patient education to improve outcomes and the patient experience.

You will triage by phone and in-office, coordinate care, manage medications, tests, referrals, and prior authorizations, and support transitions of care in a collaborative, patient-centered environment.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan.
• Valid CPR certification.
• 1-2 years of clinical experience in a medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education, and coordinate care.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
