Job summary

Phaxis is seeking a Registered Nurse specializing in Case Management for individuals with developmental disabilities in a supportive group home. This full-time role offers a Monday–Friday daytime schedule and a collaborative environment focused on resident well-being.

You will maintain MARS and health records, coordinate medical appointments, request orders, provide triage, and document care through nursing notes to ensure continuity and safety.

Qualifications

• Active RN license in the state.
• Experience in group homes or developmental disabilities preferred.
• Strong documentation and record-keeping skills.

Responsibilities

• Maintain monthly MARS and health records per OPWDD regulations.
• Coordinate and manage medical appointments with timely follow-ups.
• Request new medical orders or extensions to prevent lapses in care.
• Act as a point of contact for urgent health concerns and provide triage.
• Document nursing notes for continuity of care.
• Supervise medication passes for AMAP certifications.

Key Responsibilities

• Ensure the well-being of residents by diligently updating and maintaining their monthly Medication Administration Records (MARS) and health records, in accordance with OPWDD regulations.
• Coordinate and manage medical appointments for individuals, ensuring timely follow-ups and comprehensive care.
• Proactively request new medical orders or extensions on existing ones, ensuring no lapse in the care plan.
• Be a critical point of contact for urgent health concerns, providing triage services as needed while present in the group home.
• Conduct thorough body checks and assessments, documenting findings through detailed nursing notes to ensure continuity of care.
• Play a pivotal role in supervising medication passes for AMAP Certifications, ensuring safe and effective medication management.
