Role Overview: This position involves serving as an expanded nursing provider within an interdisciplinary team, focusing on preventative care, screening, disease management, and supporting diagnosis and treatment. The nurse collaborates with providers, patients, and families to assess needs, develop personalized care plans, and facilitate communication among healthcare team members.

Key Responsibilities

• Promote preventative health, conduct screenings, and coordinate disease management services.
• Assist with triage, patient interactions, and identifying clinical needs following guidelines.
• Educate patients and families on disease management, care coordination, and treatment options.
• Implement clinical interventions, initiate and adjust therapies, and monitor patient progress.
• Coordinate diagnostic tests, maintain documentation, and ensure accurate data entry.
• Participate in professional development, review current literature, and contribute to program evaluation.
• Handle specimen collection, medication administration, IV management, and vaccinations within scope of practice.

Qualifications & Skills

• Minimum of 2 years of RN work experience.
• Graduate of a Specialty Training Program in Nursing (required); Bachelor's Degree preferred.
• Strong clinical judgment, communication, and collaboration skills.
• Ability to utilize evidence-based guidelines and maintain accurate documentation.
