Job summary

Chapters Health is seeking an experienced Registered Nurse for night and weekend shifts to provide after-hours visits to patients, families, and community partners. You will establish and maintain positive relationships with the care team, patients, and referral resources.

This role requires a current RN license in Florida, at least one year of nursing experience (hospice or hospital preferred), and proficiency with EMR/EHR systems.

Qualifications

  • English communication required; bilingual in Spanish preferred.
  • Valid driver’s license and automobile insurance.

Responsibilities

  • Makes after-hours visits to patients in homes or facilities and provides compassionate care.
  • Assess patient/family needs and determine appropriate nursing interventions.
  • Work with the Interdisciplinary Group (IDG) to coordinate care and follow-up.
  • Educate patients and families about disease, palliative care, and safety practices.
  • Document care per policy and report changes to IDG; attend patient deaths as needed.
  • Be available for emergency coverage and extended hours as required.

Job Profile Summary

Role: The RN, Nights and Weekends provides after-hours field visits to patients, families, and community partners. Establishes and maintains positive relationships with team members, patients/families, and referral resources.

Qualifications
  • Current license as RN in the state where the employee will be working
  • Minimum of one (1) year nursing experience; hospice or hospital experience preferred
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
  • Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred
  • Mobile Driver - Valid driver’s license and automobile insurance per Company policy
  • Reliable transportation to meet patients’ needs

  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Some locations may require:Completes clinical admissions for all Company services including, but not limited to: Explains services to potential patients/families and addresses questions regarding patient fears, physical limitations, while putting the patient/family at ease; presents services in an empathetic and compassionate manner Identifies admission objections, concerns or other entry barriers and provides responses Educates patient/family on admission requirements and addresses immediate needs Completes informed consents and initial assessment according to regulations and Company policy Provides information to Physicians and initiates Plan of Care to address patient’s immediate needs Initiates skilled nursing interventions to prevent complications, alleviate symptoms and maximize physical and emotional comfort Obtains Physician orders Completes documentation per Company policy

Job Responsibilities
  • Makes after-hours visits to patients in their home, assisted living facility, nursing home, or hospital, anywhere within the assigned affiliate(s) territory. Provides compassionate customer service to patients and families.
  • Assesses patient/family and identifies immediate needs and appropriate nursing interventions by phone and/or during visit.
  • Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.
  • Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.
  • Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care.
  • Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices.
  • Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals.
  • Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support.
  • Completes documentation per policy and procedure.
  • During times of emergencies (i.e. Hurricanes, etc.), the RN, Nights and Weekends may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency.
  • Perform other duties as assigned.
Compensation

Compensation Pay Range: $67,976.53 - $106,213.33

This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
