Overview

The RN Navigator - Emergency Department (ED) is a Registered Nurse responsible for overseeing the ED Waiting Room to enhance ED throughput, patient escalation as needed, and patient experience. The RN Navigator is the “face of our ED” and collaborates with the entire health care team to impact patient movement.

Duties and Responsibilities

Duties and Responsibilities:

  • Nursing Process
    • Applies nursing process in direct patient care: provides accurate assessment and documentation of physiological, psychosocial, cultural, developmental, socioeconomic, and spiritual needs; assessment within the time frame appropriate for level of care.
    • Provides information to patient/family regarding ED care/treatment and communicates next steps along the continuum of care in the ED.
    • Adheres to current policies and procedures in patient care and seeks clarification if needed.
    • Coordinates with the multidisciplinary team to promote awareness of barriers to care and identify mechanisms to overcome them.
    • Remains a point of contact for the patient along the continuum of care in the ED.
    • Uses time management to accomplish care and follow up on delays in care or treatment.
    • Greets patients/families entering the ED Waiting Room and directs them to Registration Windows or escalates for immediate care as needed.
    • Engages with the patient/family in the ED Waiting Room and assists timely progression through waiting room steps of care/treatment.
    • Ensures vital signs are completed every 2 hours while in the Waiting Room.
    • Coordinates with providers to move treatment results to the appropriate clinicians for final disposition decisions.
    • Ensures timely discharges; assists with discharge paperwork and timely discharge.
    • Prevents Left Without Being Seen by communicating with the Triage RN and ED Provider when delays occur.
    • As needed, assists as a second Triage RN to move patients to the Provider sooner.
    • Documents patient care and follow-up in the Electronic Medical Record.
    • Assists with locating patients when called for triage/treatment/follow-up so they are not overlooked.
  • Knowledge, Skills, and Abilities
    • Knowledge of current nursing practices and procedures; interprets rules and regulations; knowledge of EMTALA laws for the ED; patient rights;
    • Acts as a liaison between the patient and the healthcare team; demonstrates leadership and professionalism.
  • Clinical and Interpersonal Competencies
    • Maintains professional attitude, works well under pressure, and contributes to crisis or emergency teamwork.
    • Adheres to privacy laws regarding PHI and maintains confidentiality of patient, hospital, and department information.
    • Effective communication with patients, families, visitors, and providers; problem solving and crisis intervention when needed.
    • Follows chain of command and conflict management processes; open to constructive feedback.
  • Age Specific & Cultural Competence
    • Knowledge of special needs and behaviors of age-specific and cultural groups; participates in related educational programs; demonstrates proficiency in required competencies.

Education, Experience, and Certifications

Education

  • Registered Nurse (RN); graduate of an accredited RN program
  • BSN preferred
  • Completes written and practical competencies annually or as assigned
  • Demonstrates knowledge of hospital information technologies used in the patient care area

Experience

  • Minimum of two (2) years ED RN experience in an acute care setting
  • Triage Certification required
  • MICN certification preferred

Licensure and Certifications

  • Current California Registered Nursing License
  • BLS, ACLS, PALS
  • MICN preferred
