The Nurse Navigator at USC Care Medical Group in Pasadena assists physicians and coordinates appointments, education, and communication across the care continuum. Requires a BSN, CA RN license, and strong communication; oncology experience and related certification are preferred. Two years of clinical/navigation experience is encouraged for this role.

Qualifications

  • Bachelor's degree in Nursing and RN license (CA) required.
  • Excellent communication skills in English are essential.
  • Knowledge of medical procedures and care coordination is required.
  • Two years' clinical experience preferred; Nurse Navigator/Oncology experience preferred.

Responsibilities

  • Initiates and coordinates patient/family teaching on diagnosis, treatment, meds and trials.
  • Reviews medications and documents medication history.
  • Evaluates patients in clinic and provides nursing care as needed.
  • Communicates with treating physicians, patients and insurers.
  • Oversees patient flow and scheduling across the continuum of care.
  • Coordinates consults with social, nutritional, case management, transportation and radiology.
  • Prepares records for MD evaluation and clinics; gathers reports and imaging.
  • Provides triage for patient care concerns and serves as a liaison.
  • Monitors lab results and follows up as appropriate.

Must have strong communication skills.Essential DutiesInitiates and coordinates patient/family teaching regarding patient care and follow - up including diagnosis, treatment plan, medications and medication side effects, procedures, and clinical trials when applicable.Reviews all medications and documents medication history when applicable.Evaluates patients in the clinic as applicable.Provides nursing care as needed.Communicates with the treating/attending/referring physician, patient, & insurer.Oversee patient flow through the continuum of care including test scheduling, surgical scheduling, assisting through the admitting process.Coordinates consults with physicians and appropriate services such as social service, nutritional services, case management, transportation , radiology etc.Prepares patient for visit and evaluation, gathers all needed reports, films and consults prior to clinic visit so records are available for MD consultation.Provides triage for patient care concerns and serves as a resource to team members. Triages phone calls from patients and caregivers.Responsible for patient contact and communication liaison between patient and health care team.Views, follows and evaluates laboratory results.Perform other duties as assigned.Required Qualifications:Req Bachelor's degree NursingReq 2 years Two years clinical experience required.Req Must have excellent communication skills, including the ability to speak, read and write English proficiently.Req Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures, in accordance with administrative and clinical policies, regulations and procedures.Req Proficient with computers, Microsoft Office Suite (Word, Excel, Power Point).Preferred Qualifications:Pref Master's degree In nursing field.
