Ochsner Health in Louisiana seeks a Care Coordinator to serve as a liaison among patients, caregivers, physicians, and the care team, ensuring smooth transitions and effective discharge planning across the continuum of care.
The role requires an RN licensed in the state, 2 years nursing experience, BLS certification, and the ability to work a flexible schedule, travel between facilities, and advocate for patients throughout the treatment process.
As a Care Coordinator, you serve as a liaison and communicator with patients, caregivers, healthcare providers, and multidisciplinary team members, facilitating seamless transitions across the continuum of care and collaborating closely with providers to plan and implement effective discharge plans.
Education
Required – Graduate of an accredited school of nursing.
Work Experience
Preferred – Associate’s or Bachelor’s degree in nursing.
Required – 2 Years of Nursing Experience.
Certifications
Required – Current registered nurse (RN) license in state of practice.
Basic Life Support (BLS) from the American Heart Association.
Preferred – Certification in Clinical Specialty Area.
Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. The job involves walking or standing to a significant degree and may require pushing/pulling of arm or leg controls. Duties may expose the employee to blood, body fluid and tissue and to hazardous medications or waste. The incumbent works in a patient care area and may be exposed to communicable diseases.
