Experience caring for patients with complex chronic disease is highly preferred.
This is a full-time nurse position working Monday through Friday from 8 am to 5 pm.
As our RN Navigator, you'll be the trusted connection between patients, providers, and care teams. Your day will involve assessing patient needs, reviewing clinical information, coordinating care plans, preparing charts for consultations and procedures, and ensuring every patient is clinically ready for the next step in their journey. You'll help create a seamless experience that allows patients to feel informed, confident, and cared for from start to finish.
