Trinity Health in Illinois is seeking a PRN MedSurg/Tele Nurse for day shifts. This part-time role focuses on patient assessment, care planning, and collaboration with the health care team to deliver safe, evidence-based care.

The nurse provides education, discharge planning, and supports continuous quality improvement as part of a compassionate, service-oriented workplace. Fourteen 12-hour shifts per six weeks, with three weekend shifts and holiday coverage as described.

Qualifications

• Bachelor's degree in nursing.
• 1-2 years of previous job-related experience.
• Basic Life Support (CPR).
• Valid Illinois driver's license.
• Current Registered Nurse license State of Illinois.

Responsibilities

• Patient Care & Assessment - Conducts comprehensive initial and ongoing assessments of patient and family need for service.
• Plan of Care - Develops and revises patient plan of care with the team.
• Coordination of Care - Documents and communicates to meet patient and regulatory requirements.
• Quality Improvement - Supports quality and cost outcomes.
• Discharge Planning - Identifies discharge planning needs and notifies patients/families.
