You will serve as the primary coordinator of all disciplines involved in patient care, ensuring effective communication and discharge planning. This position requires strong interpersonal skills and a commitment to patient well-being, working in a dynamic environment from Monday to Friday, 19:00 to 07:30.

Job description

• Shift timing: 19:00-07:30 Hours Monday to Friday (3 Days/12 Hours)
• Organization Name- Ascension Health
• Per Week: 36 Hours.
• Implement and monitor patient care plans. Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.
