Job summary

Qualifications

Responsibilities

• Assess psycho-social, spiritual, and cultural needs to develop discharge plans.
• Evaluate physiological and environmental factors to support healing and recovery.

Job Summary and Responsibilities

As our Medical-Surgical Telemetry RN, you will serve as a vital member of our acute care team, providing specialized, high-acuity nursing care to patients with complex medical conditions. You will be the frontline of clinical expertise, managing cardiac monitoring and delivering compassionate, evidence-based care in a fast-paced environment at St. Bernardine Medical Center.

Every day you will interpret cardiac rhythms, conduct comprehensive physical assessments, and monitor physiological changes in patients requiring telemetry. You will collaborate with interdisciplinary teams to execute complex care plans, perform critical risk assessments, and provide essential patient and family education regarding medication management and post-discharge recovery, all while utilizing advanced nursing technology to streamline patient outcomes.

To be successful in this role, you must possess robust clinical knowledge in Med-Surg and Telemetry, demonstrate proficiency in EKG rhythm interpretation, and show a genuine commitment to patient-centered care. • Comprehensive Clinical Care: Complete precise assessments, admissions, transfers, and discharges, ensuring all documentation meets rigorous hospital policy and regulatory standards.
• Telemetry & Cardiac Monitoring: Expertly monitor and interpret cardiac telemetry rhythms, identifying arrhythmias and responding rapidly to changes in patient status.
• High-Acuity Risk Management: Identify predictable patient problems and life-threatening conditions, initiating appropriate evidence-based interventions and Rapid Response protocols as needed.
• Holistic Patient Assessment: Evaluate the psycho-social, spiritual, and cultural needs of diverse patient populations to develop effective, individualized discharge plans.
• Physiological & Environmental Analysis: Recognize the direct relationship between physiological patient status and environmental factors to optimize healing and recovery.
• Support & Education: Identify and reinforce effective patient and family coping mechanisms, providing clear education to ensure continuity of care and improved long-term health outcomes.
