Qualifications

• Active, current, unrestricted RN license in the state of practice.
• Experience coordinating MDS processes or care planning preferred.
• Familiar with CMS requirements and post-acute care documentation.

Responsibilities

• Coordinate and manage MDS assessments and care plans.
• Collaborate with physicians, nurses, social workers and therapy teams.
• Ensure timely, accurate documentation for regulatory compliance and reimbursement.
• Participate in care conferences and discharge planning as needed.
