MDS Nurse Coordinator

Full Time, Day Shift

$78,000-$80,000 based on experience

Our RN MDS Coordinator is responsible for assuring that MDS assessments are completed accurately and timely in accordance with regulatory and facility policies. Must assure that triggers, CAAs and care plans associated with the MDS are calculated and appropriately implemented for each resident.

We are always looking for positive, dependable, caring individuals to join our Nursing team to meet the growing demand in our community for quality care. MDS Nurse Manager duties include:

  • Coordinates the facility’s process and scheduling for MDS activities for all disciplines involved in these processes. Works closely with Health Information Management and Finance Departments to assure overall compliance with state and federal standards.
  • Completes all MDS and Careplans as assigned, including necessary assessments/interviews.
  • Assures that resident assessments and careplans are appropriate, accurate, and completed within the appropriate time frames on assigned units.
  • Reviews monthly and quarterly Quality Measure and Resident Level Summary Reports for trends and appropriate documentation.
  • Reviews and assures that policies and procedures related to MDS completion are accurate and up to date.
  • Communicates effectively and maintains effective working relationship with others.
  • Completes all Quality Assurance Audits as assigned.
  • Reviews available Equip reports monthly and forwards concerns to the Director of Nursing.
