Job summary

The Allure Group's Crown Heights Center for Nursing and Rehab in Brooklyn, NY is seeking an experienced RN Assessment Coordinator to join our team. You will lead MDS assessments, update care plans, and coordinate case management to support high-quality resident care.

Collaborating with nursing staff, you'll assist with admissions, ensure regulatory compliance, and help maintain effective service delivery. Required are an active RN license, MDS experience, familiarity with care software, and

Qualifications

• Valid, current RN license.
• Experience in long-term care or skilled nursing facility.
• One year of experience with the MDS process.
• Familiarity with various computer software.
• COVID-19 vaccination.

Responsibilities

• Conduct and coordinate the completion of MDS assessments
• Maintain and update all care plans and assessments
• Examine and review residents’ medical history
• Inform other nursing staff personnel of any persistent issues
• Participate in the admission process while assessing prospective residents’ nursing needs and appropriate placement
• Coordinate case management process
• Ensure that the facility adheres to all regulatory requirements and standards of practice in relation to the MDS care planning process
• Cooperate with inter-departmental personnel to ensure services can be adequately maintained to meet the needs of the residents
