Description
Primary Responsibilities

The RN/LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental, and psychological well-being of each resident as determined by assessments and individual care plans.

Essential Functions
  • Observe, assess, document, and report resident condition/changes during rounds.
  • Ensure implementation of care plans as directed.
  • Administer medications as prescribed.
  • Conduct rounds with physicians and record visits in clinical records.
  • Perform narcotic reconciliation with incoming and outgoing shifts.
  • Identify significant changes in residents' conditions and take necessary actions per facility policies.
  • Oversee and monitor aides.
  • Initiate and complete Incident and Accident Reports as per policy.
  • Discharge residents following facility procedures.
  • Complete Nursing Assessment for Physician Calls before contacting physicians regarding resident changes.
  • Communicate with residents' contacts about status updates or condition changes.
  • Perform other projects or functions as assigned by the Director of Nurses or Administrator.
Qualifications
Educational/Training Requirements
  • Completed an RN/LVN program at an accredited vocational school or college.
Licensing Requirements
  • Must hold an active, good standing RN/LVN license in Texas.
Experience Requirements
  • Experience in a skilled nursing facility is preferred.
