Qualifications

• Graduate of an accredited nursing education program.
• Active Illinois LPN or RN license in good standing.
• No prior abuse convictions.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong communication, organizational, and teamwork skills.
• Ability to follow facility policies, maintain resident safety, and deliver compassionate, resident-centered care.
• Ability to work with diverse resident populations.
• Ability to perform the essential functions of the position with or without reasonable accommodation.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN/RN.
• Participate in developing and implementing individualized care plans with the interdisciplinary healthcare team.
• Administer medications and treatments as prescribed and evaluate their effectiveness.
• Maintain an environment that promotes residents' physical, social, emotional, and psychological well-being.
• Educate residents and families regarding care plans, medications, treatments, and medical procedures.
• Monitor nursing team member performance and assist with coaching, development, evaluation, and corrective action when appropriate.
• Delegate nursing tasks appropriately and ensure assigned duties are completed accurately and timely.
• Remain current on facility policies, procedures, infection prevention practices, and continuing nursing education.
• Recognize, assess, and promptly respond to significant changes in resident condition.
• Accurately document resident care, treatments, observations, and responses in accordance with facility policies and regulatory requirements.
• Serve as Charge Nurse by answering questions, resolving issues, and supporting clinical decision-making during the shift.
• Follow all facility safety policies, infection control standards, and regulatory requirements.
• Collaborate with physicians, therapists, and other interdisciplinary team members to provide exceptional resident care.
