Job summary

Winfield-Rest-Haven is seeking a licensed nurse (LPN or RN) for a PRN Director of Nursing liaison role. You will lead nursing staff, ensure high-quality elder care, and oversee shifts within a long‑term care facility in Kansas.

You will coordinate admissions and discharges, review medications, and monitor patient conditions, documentation, and regulatory compliance while mentoring staff and maintaining patient safety.

Qualifications

  • Graduate from an accredited nursing school.
  • Current valid license as an LPN or RN in Kansas with no restrictions.
  • Current CPR certification.

Responsibilities

  • Utilize PCC to identify changes in elders' conditions.
  • Conduct proper data collection and assessments.
  • Ensure UDAs are completed during your shift.
  • Ensure documentation is completed timely.
  • Foresee interventions for falls, wounds, infections, weight loss, pain, and other issues.
  • Coordinate admissions, transfers, and discharges.
  • Monitor elders throughout the shift.
  • Review medication orders for interactions and adverse reactions.
  • Ensure medications are monitored with vital signs and labs.
  • Document behaviors and implement non-pharmacological interventions before PRN meds.
  • Read and sign the CMA job description.
  • Initiate base-line care and update care plans as needed.
  • Provide compassionate care to elders and families.
  • Direct day-to-day nursing activities per regulations and guidelines.
  • Monitor survey compliance and contribute to QAPI.

PrimaryAccountabilities:

To serve elders in an honorable way by meeting their needs through thedevelopment and utilization of nursing staff. Ensuring the highest quality ofcare can be maintained at all times and properly utilizing resources throughoutthe facility and making independent decision in meeting these responsibilities.

Major Duties:
  • UtilizePCC throughout the shift to identify elders change in conditions.
  • Conductproper and thorough data collection or assessments on elders.
  • Ensureall UDAs are completed your shift.
  • Ensureall documentation is completed in a timely manner.
  • Foreseeneeded interventions to be proactive with falls, wounds, infections, weightloss, pain, contractures, behaviors, depression, isolation, and boredom.
  • Coordinateand conduct admissions, transfers, and discharges.
  • Monitorall elders through- out the shift.
  • Reviewmedication orders for unnecessary medications, drug to drug interaction,adverse reactions, and proper response.
  • Ensureall medications are monitored appropriately with vital signs, labs, oradditional documentation.
  • Mustdocument behaviors and implement non-pharmacological interventions prior togiving prn medications.
  • Ensureall medications have a diagnose and query physicians for proper diagnose ifneeded.
  • Identifyissues or emergencies and responding in a calm and efficient manner.
  • Readand sign the CMA job description.
  • Initiate48-hour base line care and update care plan as needed.
  • Providecompassionate care and assistance to elders and families.
  • Directthe day-to-day functions of the nursing activities in accordance with currentrules, regulations, and guidelines that govern the long-term care facility.
  • Monitorfor survey compliance and participate in the development, maintenance, andimplementation of the facility’s QAPI program for the nursing department.
Major Duties Continued:
  • Makedaily compliance rounds to ensure all nursing personnel are performing theirwork assignments in accordance with acceptable nursing standards. Report anyneeded findings to the director of nursing.
  • Mentorand training of new staff and provide support and guidance to all staffmembers.
  • Continueongoing training on dementia, behaviors, wounds, antipsychotic medications andleadership skills.
  • Coordinatedaily administrative duties, schedule, nursing assignments and elder care.
  • Superviseall nursing staff to ensure high quality of care is provided to all elders.
  • Ensureproper utilization of any device or equipment used in nursing.
Required Qualifications:
  • Graduatefrom an accredited nursing school or a nursing program from an accreditedcollege or university.
  • Currentvalid license as an L.P.N or R.N. in Kansas with no restrictions.
  • CurrentCPR certification.
  • Knowledgeableof nursing and medical practices and procedures, laws, regulations, andguidelines that pertains to nurse practice act and long-term care.
  • Mustpossess and demonstrate a high degree of leadership, organizational ability,communication skills, and be highly motivated to provide high quality of cares.
  • Knowledge of nurse practice act, policy andprocedures that apply to long-term care.
  • Has strong organizational and time managementskills and the ability to prioritize responsibilities.
  • Ability to define problems, collect data,establish facts, and draw valid conclusions.
  • Ability to apply mathematical concepts asfractions, division, multiplication, decimals, and formats.
  • Must be able to work overtime. Must beavailable for rotation on call as per facility protocol. Respond when thedirector of nursing reaches out to you by phone as appropriate.
Physical Demands:
  • Ability to lift, push, or pull 50 pounds.
  • Be able to walk, stand, stoop, or setthroughout the day.
  • Wear non-slid shoes, name badge, pager, walkie,and facility uniform.
Work Environment:
  • Professionaland deadline-oriented environment in a residential setting.
  • Interaction with older adults withvarying levels of ability.
  • Team environment.
  • Continuous exposure to residentswho are ill, confused, irritable and irrational.
  • Subject to hostile and emotionallyupset family members, associates and visitors.
  • Possible exposure to infections,diseases, and bloodborne pathogens.
