Greenfield Rehabilitation and Nursing Center is hiring RN/LPNs to provide compassionate nursing care to residents across all shifts. You will administer medications, assist with diagnostic procedures, and support physicians and the care team to deliver high-quality services in a family-friendly setting.

This role emphasizes teamwork, reliable documentation, monitoring of vital signs, and resident safety, with opportunities for a stable schedule and a welcoming work environment.

Qualifications

• Applicants must be licensed as RN or LPN.
• Experience in long-term care is preferred.
• Strong communication and teamwork skills.

Responsabilités

• Provide nursing care to assigned residents.
• Administer and document medications and treatments.
• Prepare for procedures and assist physicians as needed.
• Educate residents and families on care plans.
• Collaborate with the care team to plan and evaluate care.
• Assist with admissions, transfers, and discharges.
• Monitor and report patient condition and vital signs.
• Maintain safe, comfortable clinical environment.

RN/LPN Registered Nurse Licensed Practical Nurse

Duties and Responsibilities:

• Provides nursing care to patients as assigned.

• Administers and records medications and treatments as assigned.

• Prepares for and assists Physicians and others with diagnostic procedures and treatments.

• Reinforce supports and provides health care instructions to patients and their families.

• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.

• Assists in the admission, transfer, and discharge of patients.

• Observes and reports patients’ vital signs, symptoms and general condition to the appropriate individual(s).

• Records on patient charts observation of patient’s responses to nursing care rendered.

• Maintains environmental conditions conducive to the comfort and safety of patients.

• Assists with unit management and/or assume charge of unit as assigned.

Experience:

• RN/LPN Registered Nurse Licensed Practical Nurse:1 year (Preferred)

License:

• RN/LPN Registered Nurse Licensed Practical Nurse(Required)
