Job summary

Luxe at Wellington Rehabilitation Center is seeking an experienced RN Liaison/Case Manager for a 3:00 PM to 11:30 PM, Monday through Friday schedule in Wellington, FL. You will supervise care planning, assess residents upon admission, and perform rounds to monitor patient conditions.

The role requires an RN license, CPR/BLS certification, and 5+ years of nursing experience with at least 1 year in a supervisory capacity. English communication is essential.

Qualifications

• CPR/BLS certification.
• Minimum 5+ years nursing experience in a hospital, nursing home or other healthcare setting and a minimum 1-year experience as a nurse supervisor.
• The ability to read, write, speak and understand the English language required.

Responsibilities

• Supervise the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly.
• Assess residents/clients upon admission and periodically as conditions change.
• Perform rounds to evaluate patient condition.
• Provide or assist with the delivery of direct patient care when indicated, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated.

• Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers.

• The ability to read, write, speak and understand the English language required, knowledge of other language helpful.
