The RN will conduct resident assessments, develop care plans with input from healthcare professionals, coordinate with the IDT to improve health outcomes, supervise CNAs, and ensure compliance with IDPH/CMS standards.

Qualifications

• Active Illinois Registered Nurse license.
• Long Term Care (LTC) experience preferred.
• Strong, effective communication.
• Exemplary leadership skills.

Responsibilities

• Residents assessments.
• Care Plan development by collaborating with other healthcare professionals.
• Coordination with IDT to optimize health outcomes.
