Job summary

Signature HealthCARE is seeking a Registered Nurse to provide direct care to residents, including med administration and wound care, while supervising nursing assistants. You will develop and review care plans with the interdisciplinary team and ensure accurate documentation, admissions, transfers, and discharges.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist with activities of daily living and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts per policy.
• Monitor residents and notify physician of changes in condition.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete clinical documentation per facility policy and procedure.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
