Job summary

Trilogy is seeking an Integrated Health Clinic Nurse to deliver nursing services within the Trilogy integrated healthcare clinic and the broader community in Chicago. The role focuses on assessment, triage, care coordination, and patient education using motivational interviewing techniques and evidence-based strategies.

The nurse will collaborate with an interdisciplinary team to maintain quality standards and provide trauma-informed, client-centered care for clients with serious mental illness,

Qualifications

• Bachelor's degree in Nursing or higher.
• Active Illinois RN license.
• BLS/CPR certification.
• Valid Illinois driver’s license with daily access to a vehicle and $100,000/$300,000 liability insurance.

Responsibilities

• Partner with a Federally Qualified Health Center (FQHC) to deliver clinic nursing services and liaise between partner and Trilogy staff.
• Assess clients' health problems and triage for medical and psychiatric attention.
• Provide direct nursing care including health education, care coordination, and medication administration.
• Refill psychiatric and medical medications per established workflows.
• Advocate for clients by coordinating with Trilogy team and community services.
• Collaborate with psychiatric providers to support treatment plans and coordinate with external providers.
