Qualifications

• Associate's Degree in Nursing required at hire.
• Washington RN license required at hire.
• Minimum 2 years acute care experience in medical-surgical or inpatient settings.
• Minimum 1 year in care management or utilization review or completion of TIPS/Case Management Orientation.

Responsibilities

• Provide professional, comprehensive care management for at-risk patients in acute care.
• Perform assessments, planning, and discharge planning.
• Coordinate care and resource utilization across the continuum.
• Document interventions in accordance with standards and regulations.
