• Associate or Bachelor degree in nursing required.

Responsibilities

• Assess patients comprehensively and continuously.
• Plan individualized care with goals, teaching plans and outcomes.
• Implement prioritized nursing care and interventions.
• Evaluate patient progress and adjust plans as needed.
• Coordinate with physicians and care team using SBAR.
