Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a RN II Field (Field Based - Cumberland County) to perform assessments and develop member-centric care plans across the continuum of care.

This field-based role requires local travel within New Jersey, collaboration with physicians, hospitals and care teams, and meticulous documentation to ensure guideline-based, cost-effective care. A NJ RN license and relevant clinical experience are required.

Qualifications

• Analytical assessment of member's clinical need against guidelines to ensure medically appropriate care.
• Evaluates necessity and efficiency of services provided.
• Develops, coordinates and assists in implementation of members' individualized plan of care.
• Coordinates with patient, family, physician, hospital and other external customers regarding appropriateness of care.
