Job summary

Beacon Hospice, Amedisys/Optum (UnitedHealth Group) is seeking a Hospice Registered Nurse for per diem coverage in Boothbay and surrounding coast areas. As an RN Case Manager, you will assess patients, coordinate with the care team, and develop individualized plans that support comfort at home.

This role emphasizes compassionate care, on-call responsibilities, and thorough documentation. Requirements include active Maine RN license, CPR, 1+ year nursing experience, and a valid driver’s license

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• 1+ year of nursing experience.
• Valid driver’s license, access to reliable transportation and liability insurance

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned

As Per Diem RN Case Manager, you'll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient's needs.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as:

The hourly pay for this role will range from $36.35 to $54.55 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
