Job summary

Serenity Hospice Care in New Jersey is seeking an experienced RN to join as Hospice Clinical Care Liaison for Bergen and Hudson Counties. You will support referral development and strengthen relationships with hospitals, physicians, and skilled nursing facilities.

You will educate providers about hospice eligibility, coordinate timely referrals, and collaborate with admissions and the clinical team to facilitate transitions of care.

Qualifications

• Active NJ RN license required.
• Prior hospice nursing experience required.
• Strong understanding of hospice eligibility and end-of-life care.
• Excellent verbal and written communication skills.
• Ability to build relationships with healthcare providers.
• Willingness to travel Bergen and Hudson Counties.

Responsibilities

• Clinical Referral Support: identify, develop, and support hospice referral opportunities in Bergen and Hudson Counties.
• Hospice Education: educate physicians, hospitals, and facilities about hospice eligibility and services.
• Referral Relationship Management: maintain professional relationships with referral partners with timely follow-up.
• Patient Identification: help identify patients meeting hospice criteria based on clinical status and goals.
• Clinical Coordination: collaborate with admissions and clinical teams to facilitate timely evaluations and transitions.
• Documentation and Compliance: document referral interactions in accordance with privacy and policies.
