Job summary

Amedisys is seeking a Hospice Registered Nurse Case Manager to assess and coordinate patient care at home. You will partner with the care team to build and execute individualized plans, educate families, and ensure timely documentation.

This role anchors on-call responsibilities and seamless communication with the clinical team. Qualified candidates have at least one year of nursing experience, an active RN license in the state, CPR certification, and a valid driver’s license with reliable

Qualifications

• One year of nursing experience in home health or hospice setting.
• Active RN license in the assigned state(s).
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to work with care teams and document care plans.

Responsibilities

• Assess patients and collaborate with the care team to plan care.
• Make referrals to hospice disciplines as needed.
• Maintain individualized patient care progress records with accuracy.
• Record pain and symptom management changes.
• Educate patients, families, and caregivers in care techniques.
• Regularly communicate with the clinical manager and care team.
• Perform on-call responsibilities and on-call services as assigned.
• Other duties as assigned.

• $83,000 - $86,000 salaried + mileage
• Territory: Covington / Lexington / Eagle Rock - down interstate 81

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
