Wellstar Health System in Georgia seeks an RN to join our hospice care team. The role involves traveling to patient sites within Marietta/Acworth area to educate families about hospice services and assist with admission decisions.
The ideal candidate has 2 years of acute care and hospice experience, an active RN license (Georgia or Compact), and current BLS certification. Strong empathy and collaborative spirit are essential to support patients, families, and the interdisciplinary team.

Strong opportunity and calling to contribute to community hospice care in the field ( travel to patient sites within Wellstar campus areas of Marietta/Acworth/Douglasville/Austell etc )to enroll patients in hospice care .
2 years RN with hospice care preferred but will consider motivated RN's committed to hospice concept.
will travel to patient location to discuss hospice care with family and patients.

The Hospice admission RN is a licensed professional nurse who educates the patient and family regarding hospice services and the palliative approach to terminal care so that they are prepared to make an informed decision regarding agreement for hospice admission, performs the admission for the hospice patient choosing to live at their home, ALF or SNF. The admission process requires a nursing assessment and determination if the history of the disease and the patients current condition support the hospice eligibility, The admission RN reports the findings, assesses the needs, performs any necessary treatments or procedures, reviews the patients medication profile, develops the initial plan of care including the visit frequency of services needed, obtains the verbal certification and initial orders from the attending physician and the hospice medical director. Obtains agreement on the Plan of care with other members of the hospice interdisciplinary team. Must provide transportation to site of admission, may be exposed to pets and cigarette smoke. The framework for practice is steered by the WellStar Professional Practice Model, 5-Star Nursing and evidence based practice and research. Rn competencies are derived from these models and supported by the WellStar Core Competencies. It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected that the individual upholds the voice of the patient, department and system policies and procedures while supporting service excellence goals.
Educate the patient and family regarding hospice care and services leading to an informed decision for admission to hospice

All certifications are required upon hire unless otherwise stated.
Minimum 2 years acute care experience and hospice experience Preferred
